These kinds of notes should not be confused with psychotherapy progress reports, which are very detailed accounts of a patient's progress through one weekly meeting. Progress notes are more like short notes you can take with you as a reminder of what's been going on with your client. They show a week by week snapshot of what has happened during sessions, and while you can't necessarily draw any conclusions from it, you can get a general idea of how the session progressed. Keep in mind, however, that psychotherapy progress notes should never replace reports from your clinical therapist. A professional will have the best view of your client's progress and can advise you whether you need to continue with treatment.

Psychotherapy progress notes should be kept for every session. They don't have to be perfect, but they do have to be written honestly and quickly. No matter what your arrangement with your client is, you need to keep a record of all of his or her activities during each session, and you need to document everything. Whether it's a verbal report you write down on paper or an electronic record, it needs to be organized so that you can look back over it and see just what progress was actually made.

Some people worry about the accuracy of these notes. After all, psychotherapy progress notes are not as formal as medical records and should not be treated as such. There's nothing wrong with that concern, but don't let that stop you from using them. After all, no psychotherapy is ever going to be an exact science, and a client's reactions to therapy may well differ from another person's. Keeping a written record helps to ensure that there isn't a clash between what the client is saying and what you know.

Keep your psychotherapy progress notes regular, though. If you are giving yourself notes during individual sessions, make sure that they are given at the same time each session ends. That way, you can compare them to see where you may have made improvements. Also, don't be afraid to jot down anything that your client tells you during the day. These can be used as resources later on.

Even if you keep good notes, you should also make sure that you can access them quickly. This is especially important if you are sharing your notes with someone else outside of therapy. Writing things down is convenient but it's also easy to forget. Use your laptop or notebook as a way to keep track of what is said so that you can quickly access it when you need to.

If you find it difficult to come up with your own psychotherapy progress notes, there are plenty of templates available online. They will usually just ask for basic details about a client, though they may want more information in return. Many sites will give you free access to their samples, but you should be careful about using them. A good psychotherapy note taking service will offer you a guarantee that their work is secure. Check that this is the case before you use any of the samples available.

It can be very helpful to keep a journal with you as you progress with therapy. If you don't feel comfortable writing down everything that happens, you could consider making progress notes and keeping them in a special place where you can see them every day. This will also help you remember what was important and what was less important to you at the time. You will eventually come to a point where you no longer need to keep a journal to remember the details of your progress. Once you have a weekly or bi_weekly journal, you can remove it all together and start fresh.
